Whether you are visiting us for preventive care, ongoing medical needs, or a new concern, our team is here to help.

What to Expect at Your First Visit

Your first appointment is an opportunity for us to get to know you and understand your health history.
During your initial visit, we may:
  • Review your medical history and current medications
  • Discuss your health concerns and goals
  • Perform a physical exam, if appropriate
  • Order labs or diagnostic testing if needed
  • Develop a personalized care plan

Please plan to arrive 15 minutes early to allow time for paperwork.

What to Bring With You
To help your visit go smoothly, please bring:
  • A valid photo ID
  • Your insurance card
  • A list of current medications (including dosages)
  • Any relevant medical records or test results
  • Completed new patient forms (Please see the Forms tab to access our New Patient Application and more)
