During your initial visit, we may:
Review your medical history and current medications
Discuss your health concerns and goals
Perform a physical exam, if appropriate
Order labs or diagnostic testing if needed
Develop a personalized care plan
Please plan to arrive 15 minutes early to allow time for paperwork.
To help your visit go smoothly, please bring:
A valid photo ID
Your insurance card
A list of current medications (including dosages)
Any relevant medical records or test results
Completed new patient forms (Please see the Forms tab to access our New Patient Application and more)
